In order to provide your child with a flu vaccination we must have the following:

  • Completed Registration Form
  • Parent Signature
  • Copy of Insurance Card

ONLY FLU SHOTS WILL BE AVAILABLE AT OUR CLINICS FOR THE 2023-24 SEASON EXCEPT AT HIGHLAND SCHOOL AND ADVENT DAY SCHOOL.
